 application form
Okay, we know you don't need this one 'cause you're already a member, but you might have a mate who's interested ... so please, pass it on!

add an adult child to your cover
Heard about our new cover for your grown up kids? If they're under the age of 25, not married or living in a de facto relationship, then you can add them to your cover for a fraction of the cost of them taking out their own.
Available if you have our Private Hospital Cover. Use this form to sign them up.

 claim form
Use this form for all types of claims. Print out and complete the form by hand, or fill in your details on screen then print, sign and send it in by fax or mail.

 membership change form
There are two ways you can make a change to your membership:
1. fill in the membership change form and send it in to us, or
2. go to our online member centre and make the change on the spot.

 third party authority form
This is handy. It lets you  nominate someone who can make changes, ask about claims and generally manage your membership in case you're away or otherwise not able to do it yourself.

partner authority form
If you have a couple or family membership, the person nominated as the ‘partner’ doesn’t automatically get full access to the details of the membership, unless authority is granted by the primary member. You can grant a 'partner authority' by completing this form, or do it instantly in our online member centre.

 direct debit form
Your payments will never be out of date with a direct debit in place. Use this form to set up an automatic direct debit from your bank, building society or credit union account, or from your mastercard or visa. Print out and complete the form by hand, or fill in your details on screen then print, sign and send it in by fax or mail.

 salary deduction form
This one is so easy – pay for your health cover direct from your salary. Please check with your employer to make sure there is a salary deduction plan in place before you choose this option – and if they don't have one, ask them to give us a call.

student dependant registration
Your kids are covered by your membership until they're 21. After that they can stay with your cover if they qualify as a 'student dependant'. This form tells you everything you need to know.

 30% rebate form
If you're not currently receving the government rebate as a reduction in your contributions you can change your rebate option any time.
